You can eat great, exercise, and even target your workouts to tighten up the muscles in your thighs and glutes. For years, experts have told us that there is nothing to be done about cellulite—we just have to live with it. Then came surgical treatments involving anesthesia and incisions. While effective, not everyone wants to go under the knife for a cosmetic procedure.
Finally, there is a new (and FDA approved!) treatment for cellulite on the buttocks. Developed by Endo Aesthetics, it’s called QWO, and it’s an injectable treatment with no downtime—and no cutting involved.
What Does QWO Stand For?
QWO stands for collagenase clostridium histolyticum-aaes, the first (and the only) injectable treatment for moderate to severe cellulite in the buttocks that has been approved by the FDA. It’s for adult women and involves three separate injections over a 21-day time period.
How Do We Even Get Cellulite?
The causes of cellulite aren’t well understood, but researchers know it involves connective cords that attach the skin to the muscle, with fat in between. As fat accumulates in our buttocks, those fibrous cords pull down, and fat pushes up against the skin. Additionally, dermal thinning occurs, which means the soft tissue underneath is less supported. All of these things combine to cause the lumps, bumps, and dimpling known as cellulite.
Who gets cellulite, and who doesn’t? It is thought that a few factors come into play:
- Your genetics
- Your hormone levels
- Your body type
- Your skin texture
Clearly, muscle tone and weight also seem to affect whether a person has cellulite or not. However, as previously stated, even very fit people who appear to have no fat at all can still have cellulite.
How Does QWO Work?
The exact way that QWO works is still unknown, but we do have some clues. It is thought that QWO may initiate a process called ESR—Enzymatic Subcision and Remodeling. ESR involves three steps:
This subcutaneous incision-less surgery cuts the fibrous septae, those connective cords we talked about. Instead of undergoing an invasive cutting surgery, the injection itself breaks down the cords.
Fat Lobule Reorganization:
After the subcision, the fatty lumps start to reorganize and spread out more evenly.
Breaking down the fibrous cords actually simulates wounding to your body, and your body reacts like it does when it’s trying to heal any damaged area—it creates new collagen.

Does QWO Really Work?
Although the treatment has only been studied in one area of the body (the buttocks), there is strong evidence that it is highly effective. Over 1800 women have taken part in clinical studies of this FDA-approved injection, and the before and after photos alone speak for themselves.
How Long Does QWO Last?
Patients can expect results to last at least one to two years, although early studies suggested that the injections could permanently eliminate cellulite.
How Many Visits Will I Need?
If QWO is suitable for you, the typical treatment plan involves three visits, 21 days apart. Each visit can include two treatment areas. We will help you develop a treatment plan that is tailored to your exact needs.
Are There Any Side Effects or Risks?
As with all aesthetic treatments, there are potential risks. For example, if you are allergic to collagenase or another ingredient in the formula, you could have an adverse reaction ranging from mild discomfort to ( rarely)anaphylaxis.
However, the most common side effects are bruising at the injection site, an expected reaction experienced by over 80 percent of patients. Other effects can include hardness, itching, redness, burning, warmth, and swelling. These typically resolve within a few days.
What Can I Expect if I Decide To Get QWO for Cellulite?
